First posted on my twitter, the Ghost-Maker/Batman timeline as written by James Tynion IV.
In Batman #103 Batman commented that he made fun of Ghost-Maker when Ghost-Maker was 15 and we know that they met during their respective journeys around the world. So I'm assuming they met when they were 15 and hit it off. They could've have met earlier but I'm going with 15.
They became fast friends and spent a lot of time together before Bruce decided to tell Khoa about his mission. Khoa didn't approve and tried to kill Bruce causing their relationship to sour.
Then there's an incident in Morocco where Khoa did something to their mentor for which Bruce couldn't forgive. They fought it out in Ireland over the Morocco incident and over who get to learn knife welding from Tommy Tivane.

Then Bruce was in the Gobi Desert training with the Desert Kings when Khoa showed up for a fight and revealed that he trained with the Desert Kings before. I'm placing this encounter here because both of them seemed to have calmed down and the conversation is civil. I think this point might be where Khoa went to meet a young Jonathan Crane in Gotham, since Khoa mentioned that he sought Crane during his training years because he wanted to understand Bruce better.
Then there's their dramatic breakup in Argentina where Khoa tried to convince Bruce to team up with him but was rejected. Bruce went back to Gotham and they made a deal to stay out of each other's cities.
Then we get to that time Batman and Robin went to Singapore and ran into Ghost-Maker. And according to Nightwing Batman reached out to Ghost-Maker when making Batman Inc. but was rejected.
Of course before Ghost-Maker showed up in Gotham, he the was having his own adventures with his rogues and defeated Madam Midas. Then Bruce convinced Khoa to stay and help with Fear State and now he left to train Clownhunter but should be back in Batman #121.

Apart from that there's Ram V's Wight Witch story in Batman: Urban Legends #11 that ties Wight Witch's backstory with both Bruce and Khoa's, since the story shows that Bruce and Khoa met Rhea during their training years.
